AABB 2011 San Diego (9218-S) Novel Platelet Clearance Mechanisms

Objectives:

  • Describe the novel glycan-lectin mediated platelet clearance mechanisms induce by cold temperatures and by bacterial neuraminidases
  • Discuss room temperature storage cold-storage, with specific focus on platelet deterioration, bacterial growth and possible side effects of bacterial growth on platelet clearance
  • Discuss novel methods to investigate platelet quality for transfusion and how they correlate to platelet survival

Event Description: In this session, a former HHMI investigator with a focus on biological functions of protein glycosylation will present how a laboratory recently demonstrated that bacteria-derived sialidases remove sialic acid from platelets, leading to platelet clearance by hepatic asialoglycoprotein receptors (Ashwell Morell receptor), thereby mitigating the lethal coagulopathy of sepsis. Another speaker will present a second example of desialylated platelet clearance by hepatic Ashwell Morell receptors. Removal of sialic acid from glycoproteins occurs in platelets refrigerated for >12 hours and leads to the exposure of galactose moieties and removal of platelets. Possible mechanisms for desialylation of stored platelets will be discussed. A third speaker will review quality control methods of platelets for transfusion.

Objectives:

  • Evaluate different production methods and treatment technologies for platelet concentrates and their cost effectiveness
  • Discuss the use of platelet transfusions with a focus on hematologic malignancies and surgery and assess the cost of different platelet support strategies depending on the outcome measures
  • Discuss different options to reduce cost and the effect of cost reduction efforts on supply of platelet concentrates and on patient outcome

Event Description: In times of extreme financial pressure on health care providers, costs are critically important. This session will shed some light on the costs involved in producing, using and discarding platelet transfusions. Questions the speakers will try to answer are: what is the cost of platelet concentrates produced by different methods; what is the cost of providing platelets to hospitals; what is the cost of expired product; what factors drive the cost of platelet support at hospitals; how are different platelet products and their use modalities affecting patient outcome; where are the opportunities for cost savings and improvement?
